If you are unable to attend, we respectfully request you complete the proxy form to appoint Mr. Scott Kundinger, Area Vice President of Operations for Warren Funeral Home, Cemetery & Mausoleum, as your true and lawful proxy to vote on your behalf.

Importantly, please note that no action will be taken at this meeting that will impact your rights or responsibilities as a cemetery property owner in any regard.  To the contrary, this meeting is a routine administrative process only; and, your cemetery property and your existing funeral and cemetery contracts will remain valid and enforceable as written.

Notice to all present that I, the undersigned Member of Warren Cemetery Association, do hereby constitute and appoint Mr. Scott Kundinger, the current Area Vice President of Operations for Warren Funeral Home, Cemetery, & Mausoleum, as my true and lawful attorney, with full power of substitution, for me and in my name, place and stead to vote in favor of the director nominees as outlined above and upon such business matters as may be brought forward at the Annual Meeting of the Members on April 23, 2025, or at any adjournment thereof.
